Lack of supervision and inadequate training caused the accident . To prevent this they should have been taken through a safety training before being introduced to the work.

I didn`t believe myself being taken through topics such as LOTOTO, Work Permit, Machine Guarding, HAZCOM, PPEs and many others just in the level 3 of this HSE Course, it`s a very detailed course that leaves you with nothing but the best of skills needed to ensure a safe working place.
